Birth Certificates

Requesting Birth Certificates

There are two ways to request a copy of a birth certificate: by mail or in person. The State of Rhode Island charges $20 for the certificate.

  • By mail: Birth certificates can be obtained by writing to the Office of Vital Records, Room 101, Cannon Building, 3 Capitol Hill, Providence, RI 02908-5097, or to the city or town where the mother lived when the baby was born. If you send for a birth certificate through the mail, you must include a photocopy of your driver's license.
  • In-person: Requests may be made in person at any state office or at the city hall or town hall where the mother lived when the baby was born.

Please be advised that in most cases, you will receive your records more quickly from the city or town locations than from the State Office.

If you have any additional questions, please call Women & Infants Department of Vital Statistics at 401-274-1122, ext. 41488, 41482, or 41476.
